Founder & CEO
Richard Handley
DHSc, MHQS, MBA, PA-C, FACHE
Richard Handley is a board-certified and licensed PA whose four-decade career spans emergency, family, and occupational medicine, military service, and operational healthcare leadership.
His approach to medicine was shaped at Harvard Medical School, where he earned a Master of Healthcare Quality and Safety — and where his capstone took him to Massachusetts General Hospital for a year of embedded research in a hospital medical unit, mentored by an MGH physician. That research focused on systems failure, prevention, continuity of care, and how large healthcare systems can better support individualized, relationship-centered care.
Over his career, Richard has cared for tens of thousands of patients across busy Los Angeles emergency departments and other clinical settings, and has led healthcare operations supporting Fortune 500 aerospace and defense organizations. He is a U.S. Navy veteran who served four years on active duty as a PA and Medical Service Corps officer, and was honorably discharged as a Lieutenant. He is also a former firefighter.
Richard earned a Bachelor of Science in PA Practice from Keck Medicine of USC, conferred cum laude, and a Master of PA Studies from the University of Nebraska College of Medicine. He holds a Doctor of Medicine degree, conferred summa cum laude, from USAT College of Medicine. He practices as a board-certified and licensed PA — not as a physician — and his clinical work is delivered in that capacity in accordance with applicable California licensure requirements. He also holds a Doctor of Health Science (DHSc) focused on healthcare leadership and policy, and a Master of Business Administration (MBA).
Titan Arc emerged from that work: a deeply personalized model built around one-to-one human connection, contextual understanding, and long-term health trajectory rather than reactive symptom management alone. Titan Arc's care is delivered by and overseen by licensed clinicians in accordance with applicable state requirements.
Outside of medicine, Richard is also a filmmaker and musician — holding an MFA in Film & Television from Mount St. Mary's University — creative work that informs his belief that health cannot be understood through diagnostics and protocols alone.
Richard's work today reflects the convergence of those experiences — medicine, leadership, operational complexity, creativity, and decades of direct patient care — forming the foundation behind Titan Arc's individualized approach to longevity, performance, and human optimization.

Medical Director
Dr. Ryan Karmouta
MD, MBA
Harvard-trained physician-executive overseeing the clinical architecture and physician-guided strategy behind Titan Arc's individualized health systems.
Licensed across all 50 states, Dr. Karmouta brings a multidisciplinary perspective to modern longevity and performance medicine — integrating advanced diagnostics, regenerative therapies, recovery science, and personalized implementation into care models designed around long-term human trajectory rather than standardized protocols.
He earned his Doctor of Medicine from Harvard Medical School and Master of Business Administration from Harvard Business School before completing residency training in Dermatology at UCLA Health. He previously completed his transitional medical training at Harvard-affiliated Cambridge Health Alliance in Boston and graduated summa cum laude from New York University with a Bachelor of Science in Psychology.
His work at Titan Arc focuses on physician-led interpretation, individualized health optimization, and the clinical oversight required to translate complex health data into real-world implementation. His approach reflects Titan Arc's belief that meaningful health transformation requires context — biology, environment, behavior, recovery capacity, and long-term goals working together as one connected system.
In addition to his leadership role at Titan Arc, Dr. Karmouta maintains extensive experience in regenerative, procedural, and aesthetic medicine, with a clinical philosophy centered on precision, prevention, and sustainable healthspan optimization over time.

Dave Guttman
MBA
Entrepreneur, operator, and business strategist whose career has centered on building, scaling, and advising high-growth companies across multiple industries.
A graduate of The Wharton School, Dave has founded and scaled multiple successful businesses, including several eight- and nine-figure enterprises, and has been recognized as a three-time Inc. 500 entrepreneur. His work has spanned company building, executive advisory, operational strategy, and organizational growth, with a focus on creating businesses designed for long-term sustainability rather than short-term acceleration alone.
Over the course of his career, Dave has advised founders, leadership teams, and growth-stage organizations on scale, positioning, operational refinement, and strategic execution — helping businesses navigate complexity while maintaining clarity of vision and purpose.
His perspective on leadership and human performance has also extended beyond business. Through public speaking, including a TEDx talk, Dave has explored the intersection of resilience, personal growth, decision-making, and long-term trajectory — themes that closely align with the philosophy behind Titan Arc.
At Titan Arc, Dave helps shape the company's strategic direction, member experience, and long-term vision, bringing together operational discipline with a deeply human understanding of performance, longevity, and intentional living.

Co-Founder & COO
Agustina Visconti
Business strategist, operator, and marketing executive with more than twenty-five years of experience spanning healthcare, hospitality, wellness, and consumer experience.
Her work has included leadership and consulting initiatives for organizations including Humana, Hilton Worldwide, and Starwood Hotels, with a focus on customer acquisition, retention, sales systems, wellness programs, and operational growth. She has also spent decades working alongside concierge medical practices and preventative health organizations, developing patient engagement systems, healthcare marketing strategies, and implementation models designed to improve both organizational performance and human experience.
Agustina's expertise extends across product development, nutraceutical formulation, supply chain coordination, branding, printing, packaging, and operational design — giving her a uniquely multidisciplinary perspective on how ideas are translated into scalable, real-world execution.
During the COVID-19 pandemic, she played an instrumental role in the development and launch of ImHealthyToday, an early-market platform created to help essential workers and educational institutions remain safely operational during a period of unprecedented disruption.
At Titan Arc, Agustina oversees operations, implementation systems, member experience, and organizational integration, helping translate the company's philosophy into practical systems designed to support long-term health, behavior change, and human connection.
